A health maintenance organization keeps costs down by restricting the network. You choose a primary care physician who coordinates your care and provides referrals to specialists, and services outside the network are generally not covered at all except in emergencies. Premiums and out-of-pocket costs are typically lower, and the administrative friction is higher. A preferred provider organization gives you direct access to specialists without referrals and covers out-of-network care at a reduced rate, in exchange for higher premiums and usually a higher deductible.

Two hybrid structures fill the middle and are increasingly common. An exclusive provider organization has no referral requirement like a PPO but no out-of-network coverage like an HMO. A point of service plan requires a primary care physician and referrals like an HMO but does cover out-of-network care at a higher cost like a PPO. Reading the plan type alone is not sufficient, because the specific network composition matters more than the label.

What should actually drive the choice is whether the doctors and hospitals you care about are in the network, and that requires checking directly with the provider rather than trusting the insurer's directory, which is frequently out of date. Provider directory inaccuracy is a documented and persistent problem. If you have an established relationship with a specialist, are managing an ongoing condition, travel frequently, or live near a state line where care crosses boundaries, the flexibility of a PPO has real value. If your care is routine and local and the network includes your preferred providers, an HMO's lower cost is usually the better trade. Also verify whether the specific hospital and, separately, the physicians who practice there are in network, since a facility can participate while individual doctors within it do not.

An HMO costs less and restricts you to its network with a referral required for specialists. A PPO costs more and lets you see specialists directly and go out of network at higher cost. EPO and POS plans sit in between.
