What do cholesterol numbers mean?
Cholesterol is a waxy lipid your body needs to build cell membranes and produce hormones and vitamin D, and your liver manufactures most of it. Because it does not dissolve in blood, it travels packaged in lipoproteins. Low-density lipoprotein, LDL, delivers cholesterol to tissues and can deposit it in artery walls, contributing to the plaque that narrows arteries, which is why it is labeled bad. High-density lipoprotein, HDL, carries cholesterol back to the liver for disposal and is labeled good. A standard lipid panel reports total cholesterol, LDL, HDL, and triglycerides, a separate fat also associated with cardiovascular risk.
The dietary picture changed meaningfully. For decades guidance focused on limiting cholesterol in food, principally eggs and shellfish, but research found that dietary cholesterol has a modest effect on blood cholesterol for most people, and the US Dietary Guidelines dropped the specific daily limit in 2015. Saturated and trans fats influence LDL considerably more, and trans fats have been largely eliminated from the food supply through regulation. Genetics dominate for some people: familial hypercholesterolemia affects roughly one in 250 and produces very high LDL regardless of diet, and it is substantially underdiagnosed.
How to read your own numbers. Targets are not universal; they depend on your overall cardiovascular risk, which clinicians estimate using calculators incorporating age, sex, blood pressure, smoking, and diabetes, so an LDL that is fine for one person warrants treatment in another. Interventions with evidence include reducing saturated fat, increasing soluble fiber, regular exercise, which primarily raises HDL, smoking cessation, and statins where risk justifies them. Two nuances worth knowing: HDL raised by drugs has not translated into reduced events in trials, complicating the good cholesterol framing, and lipoprotein(a), an inherited particle not on standard panels, is an independent risk factor increasingly recommended for one-time testing.
LDL carries cholesterol into artery walls and HDL carries it away, which is the basis of the bad and good shorthand. Dietary cholesterol turns out to matter far less than saturated fat and genetics for most people.
