How does the placebo effect actually work?

The placebo effect is a genuine physiological response driven by expectation, context, and the ritual of treatment rather than by any active ingredient. It is not imagination. Brain imaging shows placebo pain relief activating the same endogenous opioid pathways as analgesic drugs, and the effect can be blocked by naloxone, an opioid antagonist, which is strong evidence that real neurochemistry is involved. Placebos also produce measurable dopamine release in Parkinson's patients and demonstrable changes in immune and endocrine markers through conditioning.

The limits matter as much as the effect. Placebos reliably influence subjective and self-regulated symptoms, pain, nausea, fatigue, anxiety, sleep, irritable bowel symptoms, and can affect measurable functions through nervous system pathways, but they do not shrink tumors, clear infections, or cure disease processes. That distinction is exactly why the effect gets exploited by alternative medicine marketing. Two findings complicate the picture usefully: open-label placebos, where patients are explicitly told they are receiving an inert pill, still produce benefit in several trials, which undermines the assumption that deception is required, and the nocebo effect runs the same machinery in reverse, where warning patients about side effects measurably increases their occurrence. All of this is why controlled trials require placebo arms, since without one you cannot tell whether a drug works or the expectation does.
