Alcohol is a sedative, and sedation is not sleep. It shortens the time to fall asleep and increases deep slow-wave sleep in the first half of the night, which is why it feels effective. It also suppresses REM sleep, the stage associated with emotional processing and memory consolidation. As the body metabolizes it over the following hours, the sedative effect reverses into a stimulant-like rebound: REM returns in a burst, sleep becomes fragmented, and the second half of the night is marked by frequent brief awakenings, often unremembered but measurable in sleep studies. The 3am wake-up is the signature of this pattern.

Several other mechanisms compound it. Alcohol relaxes the muscles of the upper airway, worsening snoring and obstructive sleep apnea, sometimes turning a mild case into a significant one for that night. It suppresses antidiuretic hormone, which is why bathroom trips interrupt the night. And it raises heart rate and disrupts the normal overnight decline in body temperature, both associated with lower sleep quality scores on wearable devices, which is why people who track their sleep often discover the effect before they notice it. Practical calibration: the disruption scales with dose and with proximity to bedtime, so an earlier drink with food is less damaging than a nightcap, and the common use of alcohol as a sleep aid is self-defeating because tolerance to the sedative effect develops faster than tolerance to the disruption.
